Our doctors and nurse practitioners specialize in caring for older adults
PACE-RI serves as a medical home for our participants and coordinates all medical and behavioral health care and social services. Our clinical team is made up of geriatricians and other experts at treating older adults with complex health needs. It can be hard for some of our participants to travel so, in those cases, our doctors and nurse practitioners provide care right in the participant’s home. And, because each PACE-RI participant is assigned their own primary care provider, they can be assured their provider will take the time to really get to know them and their health challenges. That connection between patient and primary care provider leads to better care and better outcomes. That’s why PACE-RI participants continue living at home longer and have fewer emergency room visits than other people with similar health conditions.

Our nurses care for participants in the community, at our clinics, and in our day centers. They handle much of our day-to-day clinical interactions with our participants and provide treatments, wound care, education, medication management, and support with kindness and compassion, mindful that everyone deserves dignity and respect. Their gentle perseverance and deep knowledge helps participants deal with health challenges that may arise with the privilege of reaching old age.

The day center at any PACE-RI site is always a welcoming place. With group activities and holiday celebrations, there is always something to do or someone to meet. This comfortable atmosphere also provides a place of quiet relaxation for days when you’d like to read a book or take a nap. There are exercise classes, card games, and painting sessions. No matter your interests or ability, we have something for you.

The average person at PACE is taking 9 prescriptions to address various health conditions. We know that list of medications can get confusing and seem overwhelming. That’s why we work directly with the pharmacy to organize and deliver prescriptions for you. Then we manage your schedule and keep you on track. It’s that easy!
Nutrition

Healthy eating for a healthy lifestyle
Healthy eating is critical to our overall health as we age. PACE- RI offers culturally sensitive, customized nutrition education and counselling to each participant. Our Registered Dietitian’s will meet with you and your caregiver, in your home or in one of our day centers, to review your medical history, and suggest recommendations for a healthy eating plan specific to your needs. Small group cooking demonstrations are offered, and nutritious meals are served at our day centers. Learn how small changes to your eating habits can help to prevent illness, lose weight, and have more energy.
Rehabilitation

Getting you back up and running
PACE-RI offers comprehensive Physical, Occupational, Speech, and Massage Therapy services to meet your individual rehabilitation needs. After an injury, a new or progressing health diagnosis, or to maximize your wellness, the rehab staff offer treatment opportunities in your home, or in one of our day centers. We provide participant and caregiver education, durable/adaptive medical equipment, and group exercise classes. Rehab services will offer the opportunity to remain or become more functionally independent in the community, so that you can continue to participate in the activities you enjoy!

For many PACE-RI participants, they need a little extra help in order to continue living at home. In those cases, we bring the team to you! We have certified nursing assistants who help participants with personal care like bathing, dressing, and other personal care. We also have homemakers who can help with household chores such as cleaning, laundry, dish washing, and organizing. These visits also allow us to have a regular check in with participants to make sure they are doing well.
Transportation

Getting you where you need to go
Getting around can be difficult if you don’t drive. Fortunately for our participants, PACE-RI’s fleet of shuttle buses and vans are on the move every day making trips to the day centers and elsewhere. Count on our transportation team to provide clean, on-time service to your appointments and back. Plus, with PACE-RI you know your driver and onboard CNA, making every ride a pleasant experience.

Each participant at PACE-RI has their own story. Our team of Master’s-level social workers takes the time to listen to their stories and understand what exactly they want and need. For some participants, that means talking about loneliness, painful prior experiences, or difficult family situations. For others, it means benefits coordination or addressing basic needs of health like food, clothing, or housing. Your PACE social worker will serve as your advocate, helping you get the care, services, and support you need. No matter the circumstances, our skilled social workers are here to help.

Each day center in PACE RI has a dedicated Life Enrichment department staffed with individuals trained to remove the barriers preventing our participants from doing what they love. With a special emphasis put on promoting movement and socialization, the Life Enrichment staff work with participants both in the day centers and their homes to keep them active and engaged. Utilizing a reflexive approach to care, Life Enrichment staff work to enable a variety of activities including music, art, games, baking, and more based on participant feedback.

Our health center team gets you the appointments and rides you need to see our network of contracted providers. We also offer specialty care onsite including dentistry, podiatry, optometry, laboratory tests, and audiology. And, because we also are your insurance provider, there are no copays or out of pocket expenses!
